We make house calls – enabling access to quality wrap-around services for homebound older adults with complex health needs while improving the quality of their lives, right at home.

Homebase Medical is expanding rapidly into the
Huntington Park, Cudahy, Downey, Paramount, Bell, Bell Gardens and Pico Rivera CA areas . We are currently searching for an experienced full-time Advanced Practice Clinicians (Physician Assistant or Nurse Practitioner) to join our expanding house call practice. You will be responsible for primary care in the home setting, Board and Care, and Assisted Living Facilities.
We are looking for a full-time provider for 40 hours/week. Any day of the week is open for scheduling. Responsibilities Include - Perform primary care in the home setting, Board and Care, and Assisted Living Facilities.
- Drive to patient locations in your assigned geographic area.
- Exhibit stellar bedside manner with the ability to communicate complex medical information with simplicity and kindness.
- Provide health education to patients. Work with patients in the prevention of illness, promotion of health, and management of patient's adjustment to chronic health problems.
- Spanish speaking preferred.
- Provide referrals to appropriate medical resources.
- Utilize our EMR system to document patient visit/interaction accurately and timely.
- Knowledge of HCC coding.
- Active participation in an on-call rotation is required.

Qualifications And Experience - Graduate of an accredited Physician Assistant program or Nurse Practitioner program required.
- Current, unrestricted PA or NP license to practice in California including DEA license.
- Minimum of 2 years of experience as an Advanced Practice Provider n primary care, hospice, palliative or urgent care/outpatient setting.
- Previous experience in a home health, hospice or other home visit setting is highly desired.
- Fluent in Spanish strongly preferred.
- Must live in the area of coverage.
- Minimum of 1 year of previous experience utilizing an EMR system with a good working knowledge of HCC coding
- Strong computer skills including MS Office and Outlook is strongly desired.
- Great verbal and written communication skills
- Current drivers’ license, auto insurance and reliable transportation
- Spanish speakers are strongly preferred.

What's in it for you?- Pay scale information: $132,000 to $212,000 is the expected total annual earnings after factoring in all forms of compensation including salary, fee-for-services (FFS), benefits (vacation and sick), etc.
- The base salary / salary range for this position is $68,640.
- In addition to the base salary, the Fee-For-Services (FFS) compensation, calculated based on the number of services performed or procedures completed, ranges from $35,520 to $147,520 per year.
