Our clinical team provides continuous support through both on-site and telehealth services, ensuring a robust safety net for every facility we partner with.
The Role:
This is a remote position with a requirement to visit facilities in Wellsville, New York, at least twice per month. As a Physician, you will provide professional medical services to residents of skilled nursing facilities, playing a vital role in their ongoing care.
Key Responsibilities:
Medical Director Services (Approximately 8 hours per month):
Provide expert medical and procedural consultation to residents, adhering to all applicable federal, state, and local regulations, including Title 42, Section 483.75(i) of the Code of Federal Regulations.
Advise the Facility Administrator on concerns and opportunities related to the facility's scope of services, including the development of future resident care programs, medical equipment, and staffing needs.
Maintain 24/7 availability for facility emergencies.
Attend Quality Assurance (QA) and/or Quality Assurance Performance Improvement (QAPI) monthly meetings at least 10 months per year.
Collaborate in the development of clinical pathways aimed at improving resident care.
Support the facility's marketing efforts as needed.
Assist the facility in working with other attending physicians, to assure the highest quality of care for the residents.
Direct Patient Care:
Provide comprehensive medical assessments, diagnoses, and treatment plans to residents.
Collaborate with interdisciplinary teams, including nurses, therapists, and social workers, to ensure coordinated care.
Utilize telehealth technology to provide remote consultations and follow-up care.
Perform onsite visits to facilities at a minimum of twice per month.
Qualifications:
Valid and unrestricted medical license in the state of New York.
Experience working in skilled nursing facilities or long-term care settings.
Strong clinical and diagnostic skills.
Excellent communication and interpersonal skills.
Ability to work independently and as part of a team.
Proficiency in using electronic health records (EHR) systems.
Ability to work as a 1099 contractor.
